Submit a New Request and "Total Triage"

 

From Tuesday 16/9/25, our practice will be introducing "Total Triage". This is an NHS-wide approach designed to make accessing care simpler, faster, and fairer for everyone. To do this we are now using a new online consultation tool from AccuRx. It is NHS approved and easy to use.

You can contact us about a medical, administrative or prescription issue by clicking below and filling out a short form.

Use Accurx

When submitting the form, you will need to answer a few short questions and we will get back to you within two working days. Please do not use this form for urgent or emergency requests, contact 111 or 999 if it is a medical emergency. The forms that are submitted will only be read during office hours, the forms will not be read on Saturdays and Sundays, or bank holidays. 

 

 

What does “Total Triage” mean?

 

It means that all requests—whether you need an appointment, a prescription, a medical letter, or advice—will now start with a short online form provided by AccuRx.

Once you complete the form, it is reviewed by our trained admin team or a senior clinician.  They will then decide the best next step for you. This might b

  •  A telephone or video consultation with a doctor

  • A face-to-face appointment with a nurse, or physicians associate

  • Advice from our pharmacist

  • Or another appropriate course of action

This helps ensure you get the right care, at the right time, from the right professional.

 

Why are we doing this?

  • Quicker access: By sorting requests at the start, we can reduce waiting times and prioritise urgent needs.

  • Efficient use of expertise: Not every problem needs to be seen by a GP; you may be better helped by another skilled member of our team.

  • More convenient: Many issues can be resolved by phone, video, or written advice—saving you a trip to the surgery.

  • Continuity of care: Because requests are triaged first, where appropriate we can arrange for you to see or speak to the same clinician who has been involved in your care before, helping build a consistent and trusted relationship.

  • Confidentiality: Some patients find it easier to share details on a secure form rather than at the reception desk.

 

What if I can’t fill in the online form?

Don’t worry—no one will be left behind.

  • If you find online forms difficult due to age, language or IT challenges, you can still call our reception team who will help you adjust to this new way of doing things.

  • Our staff will help you complete the form over the phone or at reception, so your request is processed in exactly the same way.

 

Change can feel daunting…

We understand that any change to the way you contact us can feel unsettling. But research shows that once patients get used to "Total Triage", they value the speed and efficiency it brings. Our goal is always to make it easier for you to get the care you need.
